Urologists of the Pirogov Clinic of High Medical Technologies at St Petersburg University have succeeded in preserving a single kidney in a patient, having performed a three-stage operation to remove a staghorn cystine stone from the patient suffering from congenital cystinuria.
It was a peculiar case because earlier the patient repeatedly had had the stones removed. Seven years ago, after he had his kidney with a staghorn stone removed, he suffered sepsis complicated by the lesion to the nervous system and the paresis of the lower extremities. After the operation, the patient from another region was under our constant online monitoring. Our specialists communicated with him on a regular basis, and now he is undergoing anti-relapse therapy for cystinuria.
In October, we admitted a patient from another region with staghorn stones of infectious nature in both kidneys. The stones developed as the result of the limited motor activity as the patient had his lower extremities amputated. The reason for the amputation was osteomyelitis triggered by a car accident. We removed a stone from his left kidney during a one-stage operation. The struvite stone from the right kidney was removed in two stages. After the operation, we treated the patient with antibiotics and now he feels well.
Today, at the Clinic, we are used to performing percutaneous stone removing operations − we do them almost every day. Because of that, in 99% of cases, we succeed in preserving the kidney. All leading clinics adhere to performing percutaneous stone removing for such types of operations. As a rule, surgeons perform contact lithotripsy approaching the kidney from the lumbar side through incisions no more than one centimetre wide.

These are the most severe forms of the disease, which may quite often lead to the death of the kidney: staghorn kidney stones usually develop asymptomatically, and by the time an individual goes to a doctor, the kidney anatomy may explicitly be changed. Additionally, in such a case, the kidney functions ineffectively: the chronic inflammation gradually destructs the normal kidney tissue, replacing it with the scar tissue. It is not a rare occasion when the kidney function is so depressed that it is too late to remove the stone. The only solution here is to remove the kidney.
